DIRECTIONS: Please fill out the entire form to successfully submit your application for Gas Service. Once you SUBMIT your application, you will be forwarded to the City of Lenox portal to pay online.

Step 2 – Order Info

Choose “Gas Deposit” and press continue

Step 3 – Payment Information

Choose “Credit Card” or “Electronic Check”. Make sure to put 200.00 in the “Payment Amount” and press continue

Step 4 – Review & Submit

Confirm everything you entered and then “Submit Payment”

By submitting the form below, I hereby apply for gas service for the address submitted pursuant to the following conditions:

  1. I agree to pay a deposit of $200.00 prior to service connection for gas service.
  2. I agree to pay all bills rendered by Lenox Gas System for services provided beginning on the address entered below, to the date service is discontinued.
  3. I agree to give notice to Lenox Gas System of my intent to discontinue service.
  4. I hereby verify that I have no outstanding bills with the Lenox Gas System under my current name or any other name I have used in the past, or under a current or former spouse’s name.
  5. I have or will receive the New Customer Package provided by the Lenox Gas System.
